Baystate Health is searching for a Nurse Practitioner to join Baystate Home Health in western Massachusetts.
Position Highlights
Join a new and developing community palliative care program providing patient-centered care in a supportive environment to patients and families facing advanced illness and/or end-of-life challenges.
Collaborate closely with a vibrant interdisciplinary team consisting of colleagues from Social Work, Nursing, Spiritual Care Services, Nurse Practitioners and Physicians.
Complete hospice recertification visits and participate in an after hour / weekend on-call rotation along with other on-call providers.
Knowledge and ability to provide assessment and manage symptoms of patients with advanced illness and recommend treatment as appropriate in a consultative model and in coordination with the primary care provider and other disciplines involved in the patient’s care.
Comfortable with holding family meetings, discussing goals of care with patient and families, and developing plan of care.
Work closely and collegially with the hospice and Homecare teams.

Qualifications:
Qualifications: The ideal candidate will be Board Certified/Board Eligible Nurse Practitioner. Candidates must have excellent communication and interpersonal skills, strong computer skills, and have a commitment to patient-centered care. New graduates considered, minimum of 1 year of nursing experience in a palliative care and hospice care required.
