Baystate Health, western Massachusetts’s premier healthcare provider and home to the University of Massachusetts Chan Medical School – Baystate Health, is seeking an experienced Physician Assistant or Nurse Practitioner to join our extensive network in Western Massachusetts.

Position Highlights:
• Join our inpatient palliative medicine program providing patient-centered care in a supportive environment to patients and families facing advanced illness and end-of-life challenges.
• Collaborate closely with a vibrant interdisciplinary team consisting of Social Work, Nursing, Spiritual Care Services, Nurse Practitioners and Physicians.
• Our inpatient palliative care team provide consults to patients at Baystate Medical Center in Springfield, MA, an academic medical center in the heart of the Pioneer Valley.
• Assess and manage symptoms of patients with advanced illnesses. Recommend treatment as appropriate in a consultative model with the hospital medicine providers and other disciplines involved in the patient’s care. Comfort with holding family meetings, discussing goals of care with patient and families, and developing plan of care.
• This position is full time, Monday-Friday. No nights, weekends, or call required.

Qualifications: The ideal candidate will be a Board-Certified Nurse Practitioner or Physician Assistant and have clinical experience in palliative care or geriatric settings. Candidates will have excellent communication and interpersonal skills, strong computer skills, and have a commitment to patient-centered care.
