POSITION HIGHLIGHTS:
• Join our inpatient cardiac surgery team that provides specialized care for patients on the Cardiac Surgery Step-Down floor at Baystate Medical Center in Springfield MA. Our Heart & Vascular Service Line includes heart failure physicians, vascular surgeons, cardiologists, cardiac surgeons, electrophysiologists, anesthesiologists, radiologists, and advanced practitioners across 4 hospitals and multiple community sites.
• This position involves daily rounding, patient evaluation, care coordination, and discharge planning in collaboration with our multidisciplinary team. Facilitate smooth transitions of care between the ICU, step-down and discharge settings.
• Ensure the delivery of high-quality, evidence-based care to optimize patient outcomes and prevent hospital readmissions. Serve as the primary liaison between the care team and patient/families, providing education.
• Engage in quality improvement initiatives to enhance outcomes and optimize workflow processes.
• This position is full time, Monday-Friday day shift.
• Qualifications: Physician Assistant or Acute Care Nurse Practitioner with 2+ years of experience in cardiac surgery, cardiology, or critical care.
• The compensation range listed below represents the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate’s relevant experience. No employee will be paid below the minimum of the range. Pay ranges are based on assumed full time commitment for exempt employees. Minimum= $130,500. Maximum= $165,300.
