Baystate Health, western Massachusetts’ premier healthcare provider and home to the University of Massachusetts Chan Medical School – Baystate, is seeking a Nurse Practitioner to join our organization.

Position Highlights:

• Join our community palliative care program providing patient-centered care in a supportive environment to patients and families facing advanced illness and end-of-life challenges.

• Collaborate closely with a vibrant interdisciplinary team consisting of Social Work, Spiritual Care Services, Nurse Practitioners and Physicians and hospice and homecare teams. Comfort with holding family meetings, discussing goals of care with patient and families, and developing plan of care.

• Assess and manage symptoms of patients with advanced illnesses. Recommend treatment as appropriate in a consultative model with the primary care providers and other disciplines involved in the patient’s care. Complete hospice recertification visits and participate in an after hour/weekend phone only on-call rotation.

• This position is full time. Part time and per Diem options also available. Schedule offers flexibility providing great work/life balance. Mileage reimbursement, phone and laptop provided.

Qualifications: The ideal candidate will be Board Certified or Board Eligible Family Nurse Practitioner or AGNP/Palliative Care Nurse Practitioner and have clinical experience in a home care, palliative care or geriatric setting. Candidates will have excellent communication and interpersonal skills, strong computer skills, and have a commitment to patient-centered care.

New graduates considered, minimum of 1 year of nursing experience in a home health, hospice or palliative care required.
